Is Digital Medicine right for me?
As part of Digital Medicine, we work with you to customize your experience so it's the most effective for you. Digital Medicine is one program that can help you manage multiple chronic conditions.
To join Digital Medicine, you need to be at least 18 years old and have a diagnosis of high blood pressure and/or Type 2 diabetes. There are some clinical restrictions that may affect your eligibility for the program. Visit the sign-up portal to see if you are eligible.

The Digital Medicine clinical team monitors your readings to ensure you're prescribed the best medication for you and health coaching provides support to help you build healthy lifestyle habits.
